RN Iohn Risely of Rawnson sent onc againe to me for my counsell his pain lieth now in his Cods {w^c} are swollen as big as ons hed & as hard as a borde he was wont to haue them swollen very big but then lasted but 5 houres & nowe swelled ever sinc weddensday he cannot eate sleepe or goe to ground. troubled w^th the wind Cholicke a paine in his backe st{illeg}ry Desirous to take a stro{illeg} but Doubteth he ca shall be able Decēb. filius  patre cū consensu. RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Decēber. 4. \die ☾./ h: 9. 15. ant m. 1598 his hart \is good/ & his tonge talketh cheerf but his body swelled.

RN Iohn Risely of Rawnstyn 63|7|. {sic} 67. hath bene sick on weddensday last 6 weekes sound in body but bursten this 16 weekes. he was wont to be troubled w^th a wind Cholick. nowe last of l {sic} all w^th an issue on his navell eaten in an Inche deepe but nowe growen vp againe saving that it hath a litle hole in it w^c when they went to stoppe it put him in danger of death. sent his sonne to me he kepeth his bed nowe & then payned in his back & pricking & shooting in his navell {virs} an end. bleedeth every foote & wetteth a handkerchir. RN William Berde 54 yers old of Potters Perye. sick on weddensday last keepeth his bed vp & downe can not rest. sent to me his sonne 26 of March die ☾ h. 8. 30. mi. ant m. 1599. sick at hart & stomack ill. very hot. a great stitch & cough & reaneth mutch fleune {sic}.

RN Thomas Gardner of Dunsanger of 62 yeres old y^t pissed blood & was mended by the herbꝭ y^t I sent him in a receipte. sinc y^t time had on Waly who hath w^th his Clisters Done him litle good but is swollen in his belly w^c is growen very harde. & mutch payned in his share & can make but a litle water halfe a sponefull at a time would knowe wheath^r leekly to die or live. went not to ground sinc munday last about 12 a clocke sent to me this 18 of Apr. die ☿. h. 7. ant m 1599 burneth in his yarde.

RN Will: Seere of haversham. 53 yeres old sitteth vp by the fier, very stiffe & hardly to {ground} onc in 24 houres complayneth of his body & lims & legs most in on of his sides all over his left syde ever sinc March last: first amisse not able to goe over the Dore threshall. sinc Alhalloutide. sent his wife to me Decēb. 21. die ♃. h. 12. 45. 1598. on of his legs the left wonderfully swelled. a very diseased body & hath had a greate cough.¹

RN Iohn Smithe sent againe his phisick wrought well. but his fathers not or very litle. Decēb. 23. die ♄. h. 10. 9. yet was very sick.

RN Iohn Whitaker of Newport c{illeg}|u|tler of 40 yeres olde fell sick & came to a misfortune the 2 of octob. die ☾ ca sent to me for my counsell this 16 of october die ☾. h. 10. ant m 1598. he keepeth his bed ever sinc except he be lifted.

RN Raynold Davyes of the glover of Stonystratford sent his boy w^th his water keepeth his bed most of all but sometimes riseth vppon the 11 of novemb^r it took him first, but was very ill handled w^th it vppon die ☉ sent his boy to me for my consell this 16 of novemb die ♃ h. 8. ant m. 1598 he thincketh he took it w^th a bruise he took w^th the caring of hides a yere since. 40 yeres old.

RN M^r Iohn lea sent his water at troubled with an ague. & a reume. 20 of March. die ♂ h. 7. 20 ant m. 1599. \feareth a palsye cold in his feete./

RN Lewis vpowen of Bric{k}|h|ill sick in his navell sent to me againe for former phisick did but give him 3 stooles and all Downeward. Apr 26. die ♃ h. 11. ant m 1599. water i{illeg}|ve|ry red & thick leeke puddle miserablely tormented. 1599

RN Frauncis Longvile of 10 yeres old. very sick in his body sent his water this 27 of Apr. die ♀ h. 9. 52. ant m. 1599. stiffe of his legs & armes.

RN Richard Travell of Handslop 70 yeres old {sick} this sennet by fits he had a bruise w^th a peece of tymber a twelmonth since. now vp & downe in his chest sent his sonn. Octob. 31. die ♂ h. 12. 40. m. 1598 had 2 wifes.

RN Iohn Risely of Rawnson mill. l 72 yeres old sick in L {sic} his body this 2 or 3 yeres about his \6 weekes./ he cōplayneth \of his navell/ it is sore & rūneth a|i|t reeseth at his navell leek an oake water. it looketh red & is rawe. he hath the cholick w^th all & y^t for this 7 yeres: maketh water very often. he thinketh his teeth to {stink} in his hed. & his feete to Double vnder him die ☿. h. 12. 15. 1598 he sent me a dish of oates he is a fuller. novemb. 8. 1598.

RN M^r furyes servant of Louton very extreeme sick at hart cannot abide talk a water leek the yellowe iaundes. sent to me againe this 4 of Ian. die ♃ h. 8. 52. mi 1599. his former phisick wrought but litle. never vomited but to the ground his m^r {sic} thinketh he hath the yellow iaundish. Iohn Windmill of yardly Gubbin 34 yeres old maried sick sinc munday night at wood lyeth at his chest cannot sleepe nor take any rest on his bed or any vp or Downe. sent his water reasonable good a cold taken vppō a heate. somewhat to yellowe & somewhat Costive. as red as skarlet his face a very strong lusty man \21 of/ March. die ☿ h. 8. 3|2|0 mi ant m. 1599

RN Richard Lingard this a yere & halfe old. troubled w^th an ague & w^th the wormes. & leekely to have the iandes the child is weaned sent to me by there sister. the fath^r & the moth^r boath Apr. 17. die ♂ h. 2. 30|5| p m. 1599. of Simpson a great cold.

RN Richard Māning of Hartwell 20 yeres sick this 7 weekes & can doe not doe any thing taketh him leeke an ague & when it cōmeth w^th a shaking colde & going away it maketh him as it were blind it so troubleth his hed. sent this 18 of Apr: die ☿ h. 1. 2|1|5. m pnn {sic} 1599.
