RN An Bourton of Olny 26 yeres old. sick the ever sinc Lent tooke her in her hed hart & backe maried this 7 yeres had but on Child but y^t god hath taken to his mercy. {s} {sic} it holdeth her nowe as an ague shaking her every day. & hat sweateth & is very hot after the shaking & hath as it were a thing lūbring in her chest very corpulent faynt & very feeble. hath a very painfull cough ready to Drawe vp hart & hall w^th it & fetcheth blood very short winded. fetcheth vp fleume & water as cleare as a bell. RN \Robart Stone of Turvy./ a strang disease in t|T|urvy some taken w^th a sorenes in there throate. 2 a greevous hed ach & stitch vnder there right syde. some die w^thin a weeke, some at the weekes end, some 2 weekes some 3. weekes. craveth a p^rvention.

RN Iohn Richardson of newp 48 y. had a third ague a yere & better. 5 y sinc & neaver well sinc. & now sorely payned in his right leg w^thout any swelling a bad stomack short wynded was wont to bleed mutch. & is very faynt & reareth blood & did bleed mutch at nose is aguish red & thick. very extreme faynt & weake & did swound last sonday. eateth litle./ extreme short wȳded

RN Rich Vmny otherwise Carter. about 3 quart^rs of a yere sick & taketh his hart thenc ariseth to his chest ready to stop his winde. rattle in his throote w^th fleume & water & a great aking of his hart & the bones of his arme. cannot take his winde so stopped in his chest & stomack the 16 of March. die ♀. h. 7. 10. 1599.

RN Arthur Smithe of Woulverton. 36. payned in the bodye nowe & lims & knees. & hath a great hote stomack and mutch bounde. arms this 7 yeres & hath spent mutch monye. faynt & feeble. maij 1. die ♂ h. 9. {illeg}|3|0. ant m. 1599.

RN M^r Stone of Rudgmont had a fall frō a horse & hurte his shyne w^ch bled sorely. now swelled & miserablely tormēted. w^th payne. hed ill.

RN Richard Foster of Dunsainger 48 yeres. Ian. 8 die ♂ h 11. ant m. 1600. it rūneth leeke a bloode vp into his ioyntꝭ into his armes & belly w^th a pricking & into his head & then his eyes runne & in the raines of his backe & out of his yarde matter white rūneth. the rūning of the reanes.

RN Anthony Smith for his sons Daughter 4 yeres old very exceeding sleepy can eat nothing keepeth her bed looketh pale. cōplayneth of her hed & her mouth sore & cannot swallowe. Octob. 10|1| die ♂ ♄ {sic}. 11. 25|0|. 1598.

RN William Berde 54 yers old of Potters Perye. sick on weddensday last keepeth his bed vp & downe can not rest. sent to me his sonne 26 of March die ☾ h. 8. 30. mi. ant m. 1599. sick at hart & stomack ill. very hot. a great stitch & cough & reaneth mutch fleune {sic}.

RN Thomas Gardner of Dunsanger of 62 yeres old y^t pissed blood & was mended by the herbꝭ y^t I sent him in a receipte. sinc y^t time had on Waly who hath w^th his Clisters Done him litle good but is swollen in his belly w^c is growen very harde. & mutch payned in his share & can make but a litle water halfe a sponefull at a time would knowe wheath^r leekly to die or live. went not to ground sinc munday last about 12 a clocke sent to me this 18 of Apr. die ☿. h. 7. ant m 1599 burneth in his yarde.

RN Will: Venter of Sawford troubled w^th a Cold very shortwinded & payned w^th a Cough & his hart sore. 31 yeres troubled w^th it this fortnight / Apr. 27 die ♀ h. 11. 15. ant m. lay first in his hed. & in his neck ready to stop his winde./
