RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN An Haman 56 of litle brickhill: sick ever sinc Christmas not hart sick but swollen in her body & in her legs & very stiffe sent to me by her husband. Martij 22. die ♃ ♄ {sic}. 1. 10. 1599. very costive & as if shee were st{oun}ge & cannot rise except shee be lifted. very cold & stiffe & short winded & leekely to die 1599.

RN Robart West of Haversham for a payne in his loynes & hips & right leg & toes ever since Sunday last at evening at 4 a clocke & hath had a winde cholicke ever since he was a batcheler & his wife also ever sinc a meaden hath bene troubled w^th a gnawing in her body. 56 yeres old. came frō Sherington. RN Cicely foster of hanslop 43 y may 20 ☾. h. 4. 39 ant m 1622 had no child this 20 y was swelled in her legs a quarter of a yere & now quite gone vpward & her belly is mutch swelled & her thigh & mutch gryped about her navell and casteth for this halfe yere y^t w^ch she taketh most cōmonly & keepeth her bed this fortnight desyreth but my prayers./

RN Goodman \Edward/ Banes of Newport 52. for a payne & ache in his knees & to his back & so to his head & the flesh about the knees is cōsumed. hot ther{s}about but cold in his feete & & benūmed about his thighes & hath scarc any feele in them. not able to goe. lyeth about the sinewes veines & hām strings & not in the flesh. the 20 of March. die ♃ h. 12. 20/ 1600

RN Ihone Nelson of Moulso of 52 yeres old. sick this 14 yere. always vomiteth vp her meate espec. this last yere. keepeth nowe her bed it g{oo}gleth in her body. y^t w^c cometh vp is black as sote nothing given her will: make her vomit shee bendeth her selfe forward & always must reatch fleume sick her back & left side & keepeth her bed. & tumbleth vp all 10. 45. ant m 1598. shee feareth lest shee be wasted w^thin. 1598. die ☿ always cold feete hands novemb. 8.
