RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN An Bourton of Olny 26 yeres old. sick the ever sinc Lent tooke her in her hed hart & backe maried this 7 yeres had but on Child but y^t god hath taken to his mercy. {s} {sic} it holdeth her nowe as an ague shaking her every day. & hat sweateth & is very hot after the shaking & hath as it were a thing lūbring in her chest very corpulent faynt & very feeble. hath a very painfull cough ready to Drawe vp hart & hall w^th it & fetcheth blood very short winded. fetcheth vp fleume & water as cleare as a bell. RN Iohn Burton of Cosgrave for his disease a boy of 9 yeres old kept his bed ever sinc tuesday. voyded wormes payned in chest belly & hart a stitch vnd^r his right \side/ a very hie coloured youth w^th the vexation y^t was pale before. all swollen frō the face to the granes. made no water sinc thursday. at 25 of March. die ☉ h. 6: 8. p. m 1599

RN Andrew Knighte of Newport 36. for the winde cholicke this moneth. & stone red sand & changeth it into blood as big as a {fe}tch somewhat yellowe. forced to \purge/ fleme yellowe Chollour in great abundanc right syde of the reanes of his back & so rūneth to the bottome of his belly. vrine hot & cōplexion w^thout the fit. RN Katheron Allen at Newport 50 yeres old. amisse this {illeg} yeres assone as shee was delivered of a child paynd in her hed. & swollen in her face. aremes {sic} legs hoven in her body sweateth & her hed & neck & syde loketh very hie & is specled. short winded & faynte. March 27. die ♂ h. 11. 30. 1599

RN Ione freeman of Newp v 17 y. very sicke. sore at the hart & hath a thing arising in her throat ready to stop her winde & was almost dead last night & cryeth & taketh & sayth shee shall dye may the 15. die ♂ h. 11. 12. m. ant m. 1599. & of her thighes. every where a pricking of her {hart}

RN Rich Vmny otherwise Carter. about 3 quart^rs of a yere sick & taketh his hart thenc ariseth to his chest ready to stop his winde. rattle in his throote w^th fleume & water & a great aking of his hart & the bones of his arme. cannot take his winde so stopped in his chest & stomack the 16 of March. die ♀. h. 7. 10. 1599.

RN fayth Gibbes of Wicken 46. amisse in her sinewes cannot gripe any thing w^th her hands so feeble & weake sometimes in hir feete & knees all swollen & keepe there naturall colour. of a good cōplex. & light harte. sent to me this 26 of Ap. h. 9. 30. ant m. 1599. a great paine in her armes. could not cut her meate.

RN M^r {sic} Charels Tutcher Charles at Stonystratford at the swan of 2 neckes. 45. sick this fortnight. sonewhat {sic} Costive shee doth bleede & is very mutch stuffed in her chest can scarce tak her winde. ready to sound a grosse & fat woman. sent a poer woman. RN Mathewe Windmill 30 yeres of age sick this two yeres in liver & milt & hart well by day working but ill always every night lieth hevy hevy {sic} in his chest y^t he cannot sleepe nor tak his rest agaynst weath^r and when he eateth flesh & porrage it heaveth vp his chest. vp from his chest to his harte. it lieth in his the pit of his stomack as if his hart & milte were broken as he him selfe thincketh it payneth him intolerable very cold never better thē when he is working cōplyneth most of chest & harte Novemb. 12 die ☉ at h. 7. 50. mi. 1598. at the first cōming he had a flushing in his hed w^t a gydines but never since

RN Iohn Ione Burten of Olney for her diseased chest 44. yeres. lame in her back. fallen downe into her legs. a great hoven in her belly body back w^th cold & a trembling ach in her back.

RN Anthony Smith for his sons Daughter 4 yeres old very exceeding sleepy can eat nothing keepeth her bed looketh pale. cōplayneth of her hed & her mouth sore & cannot swallowe. Octob. 10|1| die ♂ ♄ {sic}. 11. 25|0|. 1598.

RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Decēber. 4. \die ☾./ h: 9. 15. ant m. 1598 his hart \is good/ & his tonge talketh cheerf but his body swelled.

RN Iohn Risely of Rawnstyn 63|7|. {sic} 67. hath bene sick on weddensday last 6 weekes sound in body but bursten this 16 weekes. he was wont to be troubled w^th a wind Cholick. nowe last of l {sic} all w^th an issue on his navell eaten in an Inche deepe but nowe growen vp againe saving that it hath a litle hole in it w^c when they went to stoppe it put him in danger of death. sent his sonne to me he kepeth his bed nowe & then payned in his back & pricking & shooting in his navell {virs} an end. bleedeth every foote & wetteth a handkerchir. Decemb. 18. die ☾. h. 8. 40: mi. 1598. goeth ill to ground

RN Oliver Whiting of Bedford troubled w^th an ach in huckbone left side taketh it to be a sciatica. & thenc to his back & inside of his share came w^th a cold or a listing as he thincketh more hot & then cold. 60 yeres old. came him selfe the 28 of feb. die ☿ h. 10. 3 20|i5| ant m. 1599.
