Editorial Note : Although that note is extremely elliptical, the details of the case are given in the Ashmole 389 copy of Forman’s guide to astrology (p. 64). The details of the consultation as recorded in 389 (Harry Cook for his maid) match the text in the casebooks note, and the time and date from the 389 account match the marginal note’s placement in the casebooks manuscript.
