RN Anthony Smith of Elstoe serveth Sr Edward Ratcliffe of 60 yeres old troubled wth a cough of the lounges & lights this 12 yeres. but nowe of late very exceedingly & brought very loe. & in weake case Decēb. 27. die ♃. h. 12. 45. p m. 1599.
