Richard Napier Anthony Ballard of Elstoe 21 y May sunday before Christmas about 4 {sic} {2}|4| in the morning came to me May 2 h. 2. 30 p m 1622 satt next a fortnight. had     is lame in boath feete {illeg} his ankle is gone out very mutch. worst in h{er}|is| right leg. wth lamenes & ach in the bottone {sic} of his feete & goeth very straddlingly & pitifully
