A greiuous Headache takes him at Midnight sometimes in one te of y^e head & sometimes in y^e other te, but especially in y^e left side of his heade, w^ch makes y^e places outwardly sore where y^e paine hath beene.¹

A continually Noyse in his head w^th a tearinge paine as yf it would pull one skyn from another, w^ch comes sometimes suddenly like a strech, & then is gone in an instant

A dizines in y^e heade & a leapinge, as it were of blacke mothes before his Eyes

This paine hath continued these 6 weekes,

Desires to knowe whether it ceeds from y^e stomack or spleene, from a cold or{illeg} hote cause

Hath beene let Bloud in y^e right arme, & vnder the tongue, & in y^e forehead, beeinge tolde by the Physitian y^this greife was caused by fullnes of Bloud

Hath taken Pills & Electuaryes sundry times, but his paine noethinge deminished

Hath for many years beene troubled w^th a soundinge & noyse in his head, but neuer w^th paine till nowe Feareth a Disposition to the Palsye or Apoplexye Desires Receits & Directions to preuent these two capitall Dangers.

RN Iohn Fild of Hav^rshā 33. y. a wife & 2 child a Carpenter but hurt w^th his owne ax & hath pay cost him mutch for his Surgery. Iuly. 23 ☿ h. 9 8. 45 ant m 1628

RN Thomas Gardner of Dunsanger of 62 yeres old y^t pissed blood & was mended by the herbꝭ y^t I sent him in a receipte. sinc y^t time had on Waly who hath w^th his Clisters Done him litle good but is swollen in his belly w^c is growen very harde. & mutch payned in his share & can make but a litle water halfe a sponefull at a time would knowe wheath^r leekly to die or live. went not to ground sinc munday last about 12 a clocke sent to me this 18 of Apr. die ☿. h. 7. ant m 1599 burneth in his yarde.
