RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Decēber. 4. \die ☾./ h: 9. 15. ant m. 1598 his hart \is good/ & his tonge talketh cheerf but his body swelled.

RN Mathew Knigh|ft|en of Lowton 53. y. sick this satterd. last 3 weekes. a red water & full of rags.

M^r Knifton of Lowton 55 yeres old sick this 3 weekes {illeg} sunday last first in his stomack w^th an extreeme cold w^c tooke a way his stomack ache in the head neck & legs sweateth div^rse times & after troubled w^th an itch as exceeded greatly onc in two or 3 days to grounde. nev^r vomiteth. the 3 of Ian. die ☿ h. 9. 15. 1598. {sic} 1599. bad head greefe taken {in} lawe.

RN Elisabeth Clarke 30 yeres vnmaried ever sinc Christmas hed backe chest a few on satterday on sunday had not her Courses currantly sinc Christ. sick at hart hoven in her chest. shoulders & hed. casteth black geare leek browne black paper leeke rags y^t is her flowers arising. Apr. 3 die ♂ h. 9. 15. 1599. cannot rest in her bed hath a wonderfull red colour & is hot.

RN Iane Smithe 30 yeres old sc of haversham sick this sennet. scoureth mutch this 2 days. Dry, will nothing mouth sore water high coloured. well blooded woman Novemb. 30. die ♃ h. 9. 1598. h. 9. 18. m. ant m. 1598. a stitch vnder her right side. shee sucketh her child mutch payne in her hed & cannot sleepe.

RN Agnes Astill: otherwise Miller of Newport 40. yees {sic} old miserablely handled in her lims & ded poer & lowe brought. sent her this 26 feb: die ☾. h. 9. 15. mi. 1599. burneth & is full of blisters & itches.

RN Margaret M^res Hulles mayd 15 & M^r hulles somne {sic} 3 yeres old had lately the purpels & nowe full of spots blew, red & sanguine in his tempels face & browes. on day well. 2 days sick. a white water. Apr. 19 die ♃ h. 9. 15 1599. & {sic}

RN Richard leach of Dunsanger 64. sic not sick but hath a {gr} somewhat lyeth about his harte & hath a great cough. & when it ariseth about his harte. is ready to stop his winde & ratleth in the throate. A {sic} Octob. 2. die ♂ h. 9. 15 p ant m. 1599.

RN Amy Dēmet of Kempson 36. for a payne in her head & eyes 8 weekes. bloodshotten. itch. Ian 1. die ♂ h. 9. 15. ant m 1599 1600

RN Alce Denton of haversham 36. maried this 10 yeres 3 children last childe had a yere & more agoe feareth lest shee have taken a cold w^th a nurcing child an old Disease ever since shee was a servant a great payne at her harte, doth vomit ever sinc shee was maried {b} twice or thrice in a yere but very mutch this last weeke & never well vntill shee have it vp, els very sicke. a pert woman. sent to me by her husband this 17 of Decēb. die ☉. 9 – 18.. 1598.. best a bed.

RN vrsula Thorpe of Stony stratford keepeth the bull. hipwell vnkle vnto Thorpe 32 yeres old sick ever sinc mikelmas goeth abroade. Dece {sic} sent by M^r Oliv^r Barker. for his wife sister is her moth^r in frendship & kindred Doubteth wheath^r shee be bewitched & hath a canker in her tongue & tented shee hath an ill stomacke & spitteth putrifyed bloode. RN Anna Gascoyne young M^res Blundels sister 24|6|. y. vnmaried. Ian. 14 die ☾ hor. 9. 15. ant m. 1600 in her left syde vnder her ribs w^th a swelling & a stitch arising & heavinge vnder her syde.

RN M^r Pigget of Bechington 5|6|0 y. hath an ache fallē into his toe w^ch is swelled therwith. & is as big as boath tooes & looketh red & glistereth. Aug. 20 die ☾ h. 9. 15 ant m. 1599.

RN Ralphe Peache of Milton 34. y. for a payne & a gryping about the chest septemb. 29 die ♄. h. 9. 16. m. ant m. 1599

RN Goodman Rive did send his water to Stanton to knowe where his disease did lay what it was & where he were beg leekly to amend Iulij 17. die ☾ h. 9. 25 mi. ant m
