RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Drinkes losing him y^t hath had taken a diet losing Drinke for 3 days fasting & had 2 cakes & hath in a day. his excrementꝭ looke white & his face & eyes all yellowe but a fully faced youthe. 24 of Apr. die ♂ h. 9. 30 p m {sic}. 1599. cathed it w^th a wet & cold. faynt & feeble.
