RN Thomas Williams 30 yeres old at Wootton Vnderwoode hath a face swollen on the left syde a sive maker of a good couler: {f} {sic} yellow beard & bloody face. hath had it a yere. some call it St Germanes evill. he hath had it this yere & halfe. came to me to holpen {sic} by gods grace this 6 of Apr. being good friday die . h. 7: 3 45. 1599.
