Iohn Holmes clarke of the citchin 32|4|. y. Ap. 7. ☾. h. 11. 30 ant m 1617. serv to S^r Charles. Ap 1617. hath had an ague sinc the 12 of septēb. to the first of march. sinc an extree {sic} cold w^ch still cōtinueth w^thout remedy. hed & stomack payned every morning his belly is swelled. as if it would breake. {illeg} {sic} he tooke physick the 26 of march & therfore d|b|eing very loose desyreth no physick.

hath lost her cōplexion & looketh very pale but when she rydeth had a great cold & cough 3 quaters of a yere a payne in the mould of her head loveth any thing bett^r then meate.

first vnfoldeth his owne greefe how he is wrōged & cosened of all y^t he hath & therby he his wife & childrē broght to great want & misery craveth my Counsell & instruction
