RN Phylip Hill of Chackmore 49 y maried decēb 19. ☿ h. 1. 20|5| p m 1604 long sicke. 25 yeares a great payne in her syde came as they suppose by the stroake of a horse./

RN Iohn Richardson of newp 48 y. had a third ague a yere & better. 5 y sinc & neaver well sinc. & now sorely payned in his right leg w^thout any swelling a bad stomack short wynded was wont to bleed mutch. & is very faynt & reareth blood & did bleed mutch at nose is aguish red & thick. very extreme faynt & weake & did swound last sonday. eateth litle./ extreme short wȳded
