RN Susan Blundle at x yeares old at the 17 of Septemb. last was on the 29 of Ian at night sore troubled. & in amaner trice ded in a sownd & is every satterday so troubled at night specially Susan Blundle in her bead & sometimes in the morning. it first tooke her in the 23 of {sic} 29 day of Ian. at midnight in 1597. th 29 of Ianary. |after midnight. 30 mi.|

RN An Middleton 10 yeres old a week afore Miklemas sick miserablely handled w^th her fits. the 20 of decemb^r die ☿ at 6 in the morning. borne the 12 of Decēb. 1588. die ♃ betwixt 1. & 2 morning. her fathr {sic} enquireth what may be the cause of her disease. in her owne p^rsence. the 5 of feb: die ☾. h. 7. 30. ant m. 1599. of a white pale colour leane wellooking water very good. every day many fits. & on friday & weddensday. it maketh her knock stamp strick her hands a great cough /this figure I set in there p^rsence.\

RN Susan Blundle {illeg} 1597 was grevously handled w^th a disease w^ch taketh her every saterday since it began to take her w^c was not vntill this yeare 1597 & towards the latter end of Ian. shee was greeved therw^th on feb. the 12 of feb{r} betwixt on & two in the mor:¹ shee is as on dead & sowneth 2 or 3 times togeath^r. or a litle after midnight betwixt 12. & on. it tooke her cōmonly in her sleepe./

RN Frauncis Engolsby of Lenborowe. {illeg}|a|bout i0. sick this 5 weekes weddensday or thursday next keepeth his bed ever sinc almost & could nev^r goe fath^r sent his water by his master. Aprill 7. die ♄. h. 5. 45. m p m. 1599 cannot take his wind lyeth on his left syde but cannoot {sic} on his right syde.

RN Maria Tavernour of Marsū. 10 yeres old. Octob. 10 die ☿ h. 12. 8. 1599. |Mary Tavernour her lungs & throat cough.|

RN Edward Vuedale the sonne of Iohn Vuedale & Agnes Croutch was borne 1586 the 18 of Septemb being sonday in the mornng betwixt 3 & 4. at 3. 45 min: he is mutch troubled w^th a bunch swelling in the neck & subiect to mutch breaking out & sores legs & neck specially that it hath bene lanced. a small leane childe w^c batteleth not.

RN Iohn Blundell came for my counsell because his eyes are dim & his face breaketh out the 13 of August. h. 1. 7. m. post m 1598. of 10 y

RN Frauncis Longvile of 10 yeres old. very sick in his body sent his water this 27 of Apr. die ♀ h. 9. 52. ant m. 1599. stiffe of his legs & armes.
