RN Elis Harly of Oakely a yers {sic} old Ian. 4 ☾ h. 10. 30 ant. m 1618 {sic}.

It swoundeth often 5 tȳes a fortnight or a weke betwixt |teeth feared|

onc in a month or 3 weekes it swoundeth & then sleepeth & still is mery.
