RN Susan Blundle at x yeares old at the 17 of Septemb. last was on the 29 of Ian at night sore troubled. & in amaner trice ded in a sownd & is every satterday so troubled at night specially Susan Blundle in her bead & sometimes in the morning. it first tooke her in the 23 of {sic} 29 day of Ian. at midnight in 1597. th 29 of Ianary. |after midnight. 30 mi.|

RN Susan Blundell had a fit about 1 post m. in my howse her moth^r demaundeth the cause therof & about that time it taketh her in the day time. & about 6 a clock at night. Aug. 20 die ☉ h 1. p m. 1598.

RN M^res Blundell sent to me to come to Her daughter Susan about 12 yeres old who layd speechles for a weeke & after wardꝭ spak a worde or two. who h is troubled day & night w^th diverse fittꝭ & hath falleth often & nowe hath no vse of hed necke legs handꝭ or {sic} an but only of her eyes & yet eateth & hath a good colour. 1598 Ian die 20. die ♀ h: post merid. 1. 45.

RN Susan Blundle {illeg} 1597 was grevously handled w^th a disease w^ch taketh her every saterday since it began to take her w^c was not vntill this yeare 1597 & towards the latter end of Ian. shee was greeved therw^th on feb. the 12 of feb{r} betwixt on & two in the mor:¹ shee is as on dead & sowneth 2 or 3 times togeath^r. or a litle after midnight betwixt 12. & on. it tooke her cōmonly in her sleepe./
