RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN Robart West of Haversham for a payne in his loynes & hips & right leg & toes ever since Sunday last at evening at 4 a clocke & hath had a winde cholicke ever since he was a batcheler & his wife also ever sinc a meaden hath bene troubled w^th a gnawing in her body. 56 yeres old. came frō Sherington. RN Mres fayth Gybbes 50 sent to me againe the 5 of May. for a stiffnes in t & swelling & knobbes in t her wrest & inside of her wrests & hands, for a paine in her sinewes an {sic} an ache in her bones, hands & knees. altering from place to place. die ♄. h. 10. 30 m ant m. 1599.
