RN Margaret Tarelton \Thorne/ the wife of Will Tarelton a tealer \of Newport./ 40 yeres old 16 yeres maried sick & hath had her disease growing vppon her this 12 monets {sic} & better quærit novemb. it taketh her every foote. Novemb. 13 die ☾. h. 12. 6. mi 1598. shee was so weak shee could not come but sent her servant mead it ariseth from her hart vpward to her throot ready to stop her winde

RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Decēber. 4. \die ☾./ h: 9. 15. ant m. 1598 his hart \is good/ & his tonge talketh cheerf but his body swelled.

RN M^res Potter of willing sent for her husb. w^th out his consent at 10. 10. ant m 1599 1600 cannot sleepe his water betwixt yellow & red thick leeke puddle. his stomack hot & his tounge scab & broken out & his head cannot move but as he is turned.

first vnfoldeth his owne greefe how he is wrōged & cosened of all y^t he hath & therby he his wife & childrē broght to great want & misery craveth my Counsell & instruction
