RN Iames Greenewood of water eaton Doct^r Sparkꝭ tennet {sic} 50 yeres old. complayneth of his of h an old dysease of his harte now well & lusty but not long sinc almost ded in it. troubled w^th a stitch on his right side & cometh over his hart ready to swounde. very cold w^th it. of a high colour. stuffed in his Chest y^t he can scarce tak his winde quærit Decēbr. 2. die ♄. h. 10. 15. mi. 1598. his hart trembleth muday {sic} last sick lame.

RN Mary Anslowe of Newport 15 yeres old. M^r Mathewe Anslowes Daughter troubled w^th an ague hot & cold it taketh her every other day. & it holdeth her halfe a daye & taketh her in her bed. co every day onc to ground but it is hard ever sinc friday was fortnight. 27 of March. die ♂ h. 10. 15. m ant m: 1599. troubled mutch with fleume.

RN Thomas Leedes hath a great payne greeping his guts doubteth he tooke it w^th a fall on his ribbes. came to me for my Comsell {sic}. 75. maye the 10. die ♃ h 10. 12. ant m. 1599 cold in the morning but very hot after noone./

RN M^r Longvile of {sic} Brodwell of Woulverton for his disease as he had a fever. 26 of March die ☾ h. 10. 30. ant m. 1599 a fayntnes & panting of the hart

RN Oliver Whiting of Bedford troubled w^th an ach in huckbone left side taketh it to be a sciatica. & thenc to his back & inside of his share came w^th a cold or a listing as he thincketh more hot & then cold. 60 yeres old. came him selfe the 28 of feb. die ☿ h. 10. 3 20|i5| ant m. 1599. RN Iane Osburne of Olny. at Ap. 2. die ☾. h. 10. {illeg}|53|. ant m. 1599.

Iane Osburne of Olny of 35. for a sore back & hart best in the her bed. & nowe taketh her by fits. & burning & sweating. & aches y^t it maketh her sweate shee feareth lest it be her sicknes y^t lyeth at her harte. goeth hard to ground. her child half a yere old. had not her courses nev^r sinc shee was last w^th child & soe shee is nev^r vseth to have thē but frō child to Child. looketh yellowe. Apr. 2. die ☾ h. 10. 52. m. ant m.

RN Agnes Hoton. 7 yeres on sct Valentines eve being sunday amisse in her lims & tounge & feete & hath no sence nor feeling therin at first shee did fall for 2 or 3 dizeases. now hath lost the senc & vse of her lims

RN Ihone Nelson of Moulso of 52 yeres old. sick this 14 yere. always vomiteth vp her meate espec. this last yere. keepeth nowe her bed it g{oo}gleth in her body. y^t w^c cometh vp is black as sote nothing given her will: make her vomit shee bendeth her selfe forward & always must reatch fleume sick her back & left side & keepeth her bed. & tumbleth vp all 10. 45. ant m 1598. shee feareth lest shee be wasted w^thin. 1598. die ☿ always cold feete hands novemb. 8.

RN Iohn Scriven of Hanslop. 46. a maried man had|th| a disease y^t holdeth him as an ague. & mutch Disquiteth him in his belly chest & head. very faynt & feeble & Did sweate mutch could not tary vp frō his bed a great griping in his belly Ap. 28 die ♄ h. 10. ant m. 1599.
