RN Mathewe Windmill 30 yeres of age sick this two yeres in liver & milt & hart well by day working but ill always every night lieth hevy hevy {sic} in his chest y^t he cannot sleepe nor tak his rest agaynst weath^r and when he eateth flesh & porrage it heaveth vp his chest. vp from his chest to his harte. it lieth in his the pit of his stomack as if his hart & milte were broken as he him selfe thincketh it payneth him intolerable very cold never better thē when he is working cōplyneth most of chest & harte Novemb. 12 die ☉ at h. 7. 50. mi. 1598. at the first cōming he had a flushing in his hed w^t a gydines but never since

RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. RN Clement Nash servant to Iohn Connyngham 22 yeres old tooke her first on tuesday night qu about 8 a clock at night a crick {sic} \ache/ in her baccke \& lims./ y^t maketh her stoope & goe Double. troubled w^th a great heate of her stomack y^t it maketh her lips to break out he {sic} first water made in y^e morning red. the next water of a reasonable good colour. the wench of a ruddy face well colourd & fat. quait {sic} remediū Decēb. 4. die ☾. h. 12. 15. mi. 1598. of Newport

RN Iohn Risely of Rawnstyn 63|7|. {sic} 67. hath bene sick on weddensday last 6 weekes sound in body but bursten this 16 weekes. he was wont to be troubled w^th a wind Cholick. nowe last of l {sic} all w^th an issue on his navell eaten in an Inche deepe but nowe growen vp againe saving that it hath a litle hole in it w^c when they went to stoppe it put him in danger of death. sent his sonne to me he kepeth his bed nowe & then payned in his back & pricking & shooting in his navell {virs} an end. bleedeth every foote & wetteth a handkerchir. RN Ihone Nelson of Moulso of 52 yeres old. sick this 14 yere. always vomiteth vp her meate espec. this last yere. keepeth nowe her bed it g{oo}gleth in her body. y^t w^c cometh vp is black as sote nothing given her will: make her vomit shee bendeth her selfe forward & always must reatch fleume sick her back & left side & keepeth her bed. & tumbleth vp all 10. 45. ant m 1598. shee feareth lest shee be wasted w^thin. 1598. die ☿ always cold feete hands novemb. 8.

RN Will: Seere of haversham. 53 yeres old sitteth vp by the fier, very stiffe & hardly to {ground} onc in 24 houres complayneth of his body & lims & legs most in on of his sides all over his left syde ever sinc March last: first amisse not able to goe over the Dore threshall. sinc Alhalloutide. sent his wife to me Decēb. 21. die ♃. h. 12. 45. 1598. on of his legs the left wonderfully swelled. a very diseased body & hath had a greate cough.¹
