A greiuous Headache takes him at Midnight sometimes in one te of ye head & sometimes in ye other te, but especially in ye left side of his heade, wch makes ye places outwardly sore where ye paine hath beene.1
A continually Noyse in his head wth a tearinge paine as yf it would pull one skyn from another, wch comes sometimes suddenly like a strech, & then is gone in an instant
A dizines in ye heade & a leapinge, as it were of blacke mothes before his Eyes
This paine hath continued these 6 weekes,
Desires to knowe whether it ceeds from ye stomack or spleene, from a cold or{illeg} hote cause
Hath beene let Bloud in ye right arme, & vnder the tongue, & in ye forehead, beeinge tolde by the Physitian ythis greife was caused by fullnes of Bloud
Hath taken Pills & Electuaryes sundry times, but his paine noethinge deminished
Hath for many years beene troubled wth a soundinge & noyse in his head, but neuer wth paine till nowe Feareth a Disposition to the Palsye or Apoplexye Desires Receits & Directions to preuent these two capitall Dangers.

1 Had a colde & an extreame Cough a weeke afore shouetyde {sic} wch did much shake & macerate his Body & was freed both of colde & Cough wthin 3 weekes after. findes a defect in his digestion, in the liuer as hee feareth, wch heretofore hath much anoyed him.
2. is \hath beene/ very costiue (saueinge for 3 dayes last past) wth some forcemt thereto, yet his Body is temporate for heate & colde.
3 hee is not much thirsty saue at meales;
4 hee sleepes indifferent well, 5 or 6 houres in the night at beast.
5 his stomack seemeth to bee quieted when hee suppeth posset drinke
6 This is his Clymatericall yeare, beeinge 63 yeares at Alhollowetide last.
7 hee desires to knowe howe his Liuer may bee releeved & howe his Body may be loosened.
8 His was water ordinarily keepes in a reasonable lowe Coulour.
9 Hee feares yt his Body (beeinge old & weake) will not much abide Phisike./
10 his dyet com̄only is veale & broth. & sometimes mutton, his drinke is small Be{e}re

Hee tooke not the vomite yt was sent. 1 because hee is very vnapt to vomite; 2 because his neck is longe & brest somewhat straite; 3 because vomiteinge once by chance by reason of Tobacco blowne into his mouth it brought vp Bloud wth it, & hath beene ye cause of all his infirmity. 4 hee suspecteth his Lunges, for wch hee thinkes vomiteing is very hurtfull.
Took his purge wch gaue him 12 occasions. Hee desireth a Course to bee vsed for his Brest & Lunges.
