RN Iames Greenewood of water eaton Doct^r Sparkꝭ tennet {sic} 50 yeres old. complayneth of his of h an old dysease of his harte now well & lusty but not long sinc almost ded in it. troubled w^th a stitch on his right side & cometh over his hart ready to swounde. very cold w^th it. of a high colour. stuffed in his Chest y^t he can scarce tak his winde quærit Decēbr. 2. die ♄. h. 10. 15. mi. 1598. his hart trembleth muday {sic} last sick lame.

RN Agnes Hoton. 7 yeres on sct Valentines eve being sunday amisse in her lims & tounge & feete & hath no sence nor feeling therin at first shee did fall for 2 or 3 dizeases. now hath lost the senc & vse of her lims

RN William Tomson of 12 yeres on a friday a moneth afore Miklemas quærit valde ægrotat ventre. 6 of novemb. it tooke him on 2 of novemb. die ♃ in the morning. he it cometh & taketh him by fits & & {sic} ariseth vp in his stomack he could not take his meate. but would vomit vp but y^t is stayd & troubleth his hed but most his belly burneth wonderfully cannot eate. RN Will Knight tooke his sicknes die {illeg} Ian 4 die Ian. {sic} 5. die ♀ h. 3. in aur. very evill handled w^th sicknes vomiteth hath a rising in his stomack leeke an apostume ready to stoppe his breath. burneth very mutch thirsty & drye. 1599.

RN Elisabeth Clarke 30 yeres vnmaried ever sinc Christmas hed backe chest a few on satterday on sunday had not her Courses currantly sinc Christ. sick at hart hoven in her chest. shoulders & hed. casteth black geare leek browne black paper leeke rags y^t is her flowers arising. Apr. 3 die ♂ h. 9. 15. 1599. cannot rest in her bed hath a wonderfull red colour & is hot.

RN Mary Nuby 28 of Grafton Lodge. for a rising at her stomack. her legs. Double vnd^r her faynt & feeble & pineth in her body & yet can earte {sic} had not her termes never sinc her sicknes almost for a yere. Ap. 25 die ☿ h 5. 30. p m. 1599 |cannot sleepe troubled w^th a great cough at night.|

RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN William Downing \of filgrave/ of the his child Abigayle a yere & 3 quarters. old. very sick for 3 houre{s} when it taketh it at 2 a clock morning & at 12 it vomiteth whatsoever it taketh the Drinke & sucke it taketh & handleth very ill for the space of 3 houres Aug 27. die ☉ h. 3. 1598 quærit remediū it loketh pale as ashes at p philgrave.

RN Will. Grene venit ad me quærens morbū 1597 Martij 8. die ♂ h. 12. 30 m. after noone he had a sore legg wounded & broken w^th a coggle. his principall disease his leg is swollen & ranckled.

RN Iohn Burton of Cosgrave for his disease a boy of 9 yeres old kept his bed ever sinc tuesday. voyded wormes payned in chest belly & hart a stitch vnd^r his right \side/ a very hie coloured youth w^th the vexation y^t was pale before. all swollen frō the face to the granes. made no water sinc thursday. at 25 of March. die ☉ h. 6: 8. p. m 1599
