RN Elisabeth Emmerton of Nash 2i yeres sick this sennet full of paine every where round about her back & lins {sic} & into her hands. 15 of March. die ♃. h. 9. 49. 1599. shee camot {sic} tak any rest day or night light headed

RN M^r William Chester was borne vppon Easter monday about 4 of the clock p m. this Child is f {sic} very small faynt feble of a longe visage hath not the vse of his lims but specially of his legs to goe natus āno dni 1596. 12 Apr. die ☾ h. 4 p m. 1596.

Isbell Dormer of Woulverton 28. sick this quart^r of this yere in her stomack & harte & her right side & right arm supposing y^t her liver is growing to her side. die ☾. h. 9. 2|3|0. 1599. shee doth swound w^th it worst at mornings.

RN Mres Blundell enquireth what is good for her Disease shee is pulled as it were in peeces hart belly sides & swollen in her stomach y^t & worse after she hath eaten or Drunken weake & feble in her body & knees. Aug. 28. die ☾ h. 5. 28 mi p. m shee shaketh & trembleth body {neck} & {yoin}tꝭ

RN Thomas Bartom. 15 yeres sick this 5 weekes. nothing will tary w^th him for this fortnight lyeth in his hed light headed. Martij 8. die ♃. h. 9. 10. 1599. at Newport.

his fath^r moved a question at the same time or a quart^r after 9. for a payne in his knee w^c was broken his thigh was broken quite thorough & nowe the paine is fallen into his knee pan & is swollen.

RN Isbell Linwell servant to Thomas Buncher of Paulus Perrye 30 yeres old nor fath^r nor moth^r a hie colourd wate woman & sweateth hart swollen in the body & legs. & chest faynte y^t shee can do no worke hed & hart beateth. 27 of March die ♂ h. 1. 30 p m. 1599. short winded hath bene amisse this 3 weekꝭ {wak}heth

RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN Iohn Trueman of Willing i9 come lāmas. hath a swellinge in his legs fallen downe lately w^thin this sennet his \belly & body/ was before swollen. he hath taken phisick this 4 yeres. & hath taken Drinkꝭ of many & of M^r Glover of Northampton. Drinkes losing him y^t hath had taken a diet losing Drinke for 3 days fasting & had 2 cakes & hath in a day. his excrementꝭ looke white & his face & eyes all yellowe but a fully faced youthe. 24 of Apr. die ♂ h. 9. 30 p m {sic}. 1599. cathed it w^th a wet & cold. faynt & feeble.

RN Thomas Whooton of haversham about 22. vnmaried. sick this yere. quærit remediū. novemb 6. die ☾. h. 12. 30. he hath had a swelling in his right side broken out in his wrestꝭ & body filthy water. heavy vncōfortable his hed gyddy & light his sight bad

RN Katheron Allen at Newport 50 yeres old. amisse this {illeg} yeres assone as shee was delivered of a child paynd in her hed. & swollen in her face. aremes {sic} legs hoven in her body sweateth & her hed & neck & syde loketh very hie & is specled. short winded & faynte.
