A greiuous Headache takes him at Midnight sometimes in one te of y^e head & sometimes in y^e other te, but especially in y^e left side of his heade, w^ch makes y^e places outwardly sore where y^e paine hath beene.¹

A continually Noyse in his head w^th a tearinge paine as yf it would pull one skyn from another, w^ch comes sometimes suddenly like a strech, & then is gone in an instant

A dizines in y^e heade & a leapinge, as it were of blacke mothes before his Eyes

This paine hath continued these 6 weekes,

Desires to knowe whether it ceeds from y^e stomack or spleene, from a cold or{illeg} hote cause

Hath beene let Bloud in y^e right arme, & vnder the tongue, & in y^e forehead, beeinge tolde by the Physitian y^this greife was caused by fullnes of Bloud

Hath taken Pills & Electuaryes sundry times, but his paine noethinge deminished

Hath for many years beene troubled w^th a soundinge & noyse in his head, but neuer w^th paine till nowe Feareth a Disposition to the Palsye or Apoplexye Desires Receits & Directions to preuent these two capitall Dangers.

I had good entertaynement. but came home wet over shoes. for I was Driven to wade in my house & shoes.

RN Iune 8 ♂. h 2. 20. p m hor ♂ I went to M^r Byrds & aft^r a good supper we had talke of the crosse of Ihūs christ & greate Disputation. wheath^r it ough{t} to be vsed in baptisme I or noe. they Denyed it I {ass}{u}med it they held it as a popish & a wicked ceremony. this Disputation was betwixt 8 & 9 of the clocke & about after 9 of the clocke on came to M^r Byrd{s} f|t|o me for physicke w^ch met w^th on that Directed him vppon the stone bridge & told him he should fynde me at M^r Byrds house Iune 8 ♂ hor 8. 30 p m. 1602.

I came home in y^e wet
