RN Rich Kerby Miller of Newport 60 \56/ yeres old. long & grevously sick {filiu}s  patre \{servus}  Domino/ cū euꝯ {sic} consensu it hath held this 4 yeres by fits but this quarter of this yere held him sorely & keepeth home some days his bed in his fit & then abroad. a payne in his hart w^th a sighing black coloured very fat of his face but w^th this sicknes shrunk in his bodye & legs. M^r yarrow for him novemb. {sic} 1 Decēb. die ♀ h. 10. 15. 1598¹

RN Iohn Porter butcher of S^t Albons fell sicke the 21 of Novemb. an houre \D an/ afore ☉. in his shoulders. die ♂ h. 3. 30. sent M^r Nichols in his behalfe the 23 of Novemb die ♃ h. 2. 15. mi. 1598.

RN vrsula Thorpe of Stony stratford keepeth the bull. hipwell vnkle vnto Thorpe 32 yeres old sick ever sinc mikelmas goeth abroade. Dece {sic} sent by M^r Oliv^r Barker. for his wife sister is her moth^r in frendship & kindred Doubteth wheath^r shee be bewitched & hath a canker in her tongue & tented shee hath an ill stomacke & spitteth putrifyed bloode. RN Elisabeth Rigot 18 y. servant to Cowly the Butcher sick this fortnight cōplayneth most of her hed keepeth her bed ever sinc satterday her master sent w^thout her privity the 27 of feb. die ♂ h. 8. 20 1599.
