hath a Swellinge in his Back & RN \genitalibus rūning & in his/ hand cannot hold his water Lame in his Thighe & Hucklebone

A greiuous Headache takes him at Midnight sometimes in one te of y^e head & sometimes in y^e other te, but especially in y^e left side of his heade, w^ch makes y^e places outwardly sore where y^e paine hath beene.¹

A continually Noyse in his head w^th a tearinge paine as yf it would pull one skyn from another, w^ch comes sometimes suddenly like a strech, & then is gone in an instant

A dizines in y^e heade & a leapinge, as it were of blacke mothes before his Eyes

This paine hath continued these 6 weekes,

Desires to knowe whether it ceeds from y^e stomack or spleene, from a cold or{illeg} hote cause

Hath beene let Bloud in y^e right arme, & vnder the tongue, & in y^e forehead, beeinge tolde by the Physitian y^this greife was caused by fullnes of Bloud

Hath taken Pills & Electuaryes sundry times, but his paine noethinge deminished

Hath for many years beene troubled w^th a soundinge & noyse in his head, but neuer w^th paine till nowe Feareth a Disposition to the Palsye or Apoplexye Desires Receits & Directions to preuent these two capitall Dangers.

It came after the smale pox. a full pletherick body |hath had purges pils dyet drinks & An Angle frō king Iams & also this king /her face swelleth a litle w^thin these 2 months her swelling\| |neaver had her terms| |hath bene let blood twice in her armes & twice in her feete & twice in her hands & leaches at her lips & thighs & fundament|

RN Annys Anne |a poor wench| /13 y\ of Southwick in Northāptonsheere by Owndall |M^r Lynn Esquire wrote in her behalfe| It tooke her 16. Ian 16 fell into her fits of convulsions swonding fits w^th quaking & trēbling Ian 30. ♃ h. 11 ant m 1634 taketh away her speach swoundeth convuls¹

SRN {M}^r William Luttwich of Luttwich. {D}ecemƀ 30. ♂. h 9. 45. a. m. 1634.

Complained of Cramps & weaknes ouer all his body. face, Armes legs were swelld, now thanks be to God are not. was the payne in his back doth not so much trouble him as before his loosenes stayd vpon those things I gaue as Clysters & Cordialls. now his loosenes hath taken him agayne desires something y^t may stay it. vryne a little higher y^en it hath bin this 3 dayes, no slu{ff}y Sediment, nor so much troubled with the Gonorrhæa as before Laus deo.

Desires at this tyme something for Cough & loosenes.
