RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. RN Rich Vmny otherwise Carter. about 3 quart^rs of a yere sick & taketh his hart thenc ariseth to his chest ready to stop his winde. rattle in his throote w^th fleume & water & a great aking of his hart & the bones of his arme. cannot take his winde so stopped in his chest & stomack the 16 of March. die ♀. h. 7. 10. 1599.
