RN M^r Blundell enquireth what his disease the cause therof & the remedy & he cōplayneth of the winde cholick & seemeth to be troubled w^th stone & strangury. August the 12 die ♄. h. 8. ant m. 1598.

RN Rich. Risely of Willing 2 yeres old. the fortnight after Chr|E|sterday ☉ morning about 3 a clock morning troubled by fits. especially the 18 of Decēb. after noone it wull {sic} fall Downe & crye & tak hold of any thing. Richard Risely a very feare child & of a goodly cōplexion was borne 1597. the 10 of Aprill. die ☉ h. 3. ant m. 1597. came to me Decēb. 29 die ♀ – h. 9. 30. cū vxore et filiolo. borne at willing.

RN Thomas Whitson of Shenly 50 yeres sick ever sinc yes tuesday last quærit burneth exceedethly exceedingly & his disease is stronger at some times then othr {sic}. die Octob. 4. die ☿. h. 7. 43|4|. 1598. the newe Disease y^t towne hath bene troubled mutch w^th y^t disease

RN Mathew Tedder of Newport at {M} 22^th of March next following {sic} of 39 at 22^th of March next following hath had a Disease following him any time this twellyeres. 9. 45|9| ant m Aug. 28 die ☾ h. 9. 45. {sic} 1598. he hath the rūning of the reanes w^c weakneth him so sore that he can scarce stand. his bac{k} exceeding ill. mutch troubled w^th it for this halfe yere.

RN Edmund Hauckings of Newport 47|8| yeres old came to me reasonable well for ought I could ceave amisse & not sound in body any time 10 yees {sic} commonly at this time of the yeare but taketh him at Alhollantide & holdeth him vnto the springe of the yeare. came for my consell him selfe this 15 of Decemb^r die ♀ h. 9. 54. 1598. of a high reddish Colour of a black hed & beard. this disease hath greeved him a Chilly Cold in his hed.

RN Rob Vilveny. what his disease is or to what inclined & spec. what is good for his liver of 22 y. 9 Aug. h. 2 p m die ☿ 1598.¹

RN Elisabeth Sandons the daught^r of Iohn Sandon ded & deted & moth^r. her vnkle came for her at Woulverton 9 yeres old amisse this fortnight they suppose it to have bene an ague. very sleepy & Drowsy. cōplayneth of her hed & belly. Novemb. 17. die ♀ h. 10. 15. mi. 1598.

RN Thomas Hooton for his extreeme payne in his hed & eyes leek a megrim. Novemb. 18 die ♄ hor. 10. 9. 1598.¹

RN Thomas vnderhood 20 yeres old of stratford for to break his cold for he hath a cough w^c maketh him vomit. every day when he Drinketh cold Drink he Doth vomit. Decēb. 13 die ☿ h. 2. 30|5|. p m. he is short winded

RN R N. valde ægrotabat corde capite et stomacho. quæritu^r causā. Sept. 11. die ☾. h. 3. 53 m. post m. 1598 apoplexia et verti

RN Harry Goodin taken lame in his left leg not swollen he cannot styrre his right side. in his leg & huck bone he layd in the fild Downe & being hote tooke a colde. this day or monday was sennet 17 d 16 last Christmas eve. 1598. pater  filio et filius oravit patrē. vt me consulet  morbo. Octob. 29 die ☉ h. 1. 1598. it took him in the fild as if it were prickt with nales. he cryeth out whē it is touched.
