RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN Mres Vines took her sicknes at Piddington 30 of maye die ♂ h. betwixt 9. & 10 walking at night in her orcharde. & it tooke away her speache & brought a palsy in her side & armes. shee had no vse of her lims. 1598. die ♂ h. 9. 30 mi. p m maij 30. 1598.¹

RN M^r William Chester was borne vppon Easter monday about 4 of the clock p m. this Child is f {sic} very small faynt feble of a longe visage hath not the vse of his lims but specially of his legs to goe natus āno dni 1596. 12 Apr. die ☾ h. 4 p m. 1596.

RN Williā Disny of Simpson 60 y. for a palsy in his left hand. Octob. 15 die ☾ h. 3. 30. p m. 1599

RN Iudith B{unn}e fatherles & mothrles 12 yeres. lame ever since this fortnight. frō the knees Downewarde. Apr. 24 die ♂ h. 8. 53. ant m 1599. a good water full full {sic} of scals & motes.

RN Mres Blundell enquireth what is good for her Disease shee is pulled as it were in peeces hart belly sides & swollen in her stomach y^t & worse after she hath eaten or Drunken weake & feble in her body & knees. Aug. 28. die ☾ h. 5. 28 mi p. m shee shaketh & trembleth body {neck} & {yoin}tꝭ

RN Iohn Ione Burten of Olney for her diseased chest 44. yeres. lame in her back. fallen downe into her legs. a great hoven in her belly body back w^th cold & a trembling ach in her back.

RN An Turner of Newport servant to Goodmā Commendale troubled w^th a swelling at her Chest. & in her hed. this two or 3 days. hath not the vse of her armes. 28. y. at miklemas Ap 25. die ☿ h. 4. 30. 1599.

RN Elisabeth Townsend of Newporte for an ache in her huckbone & so goeth downe into her ankle. worst when shee is hott in her bed. hath no sence nor feeling of her {b}e feete w^thout swellinge Sept: 11. die ♂ h. 1. 15 p m 1599 44. y.

RN An Goodin of Emmerton maried about 48. sinc whitsontide ill at her hart & then her back ready to teare a {sic} her harte & exceeding thirsty & Drye. Octob. 5. die ♃ h. 2. it tooke her lame in her leg

RN Mary Porter of Stadgdon 40 yeres troubled w^th an ache vppō her right should^r & arme & left thigh to her leg downeward & hath no vse of thē feb. 21. die ♃ h. 2 15 p m. 1600.

RN Goody Tealer of 70 y old of harsom taken with a deadly palsie in her syde & tounge. came to me for helpe. Ian. die ☿ h. 1. p m. Ian. 23. 1600
