Question asked by the patient on 25 March 1623 between 10:00 and 10:50

RN Iohn Richardson of newp 48 y. had a third ague a yere & better. 5 y sinc & neaver well sinc. & now sorely payned in his right leg wthout any swelling a bad stomack short wynded was wont to bleed mutch. & is very faynt & reareth blood & did bleed mutch at nose is aguish red & thick. very extreme faynt & weake & did swound last sonday. eateth litle./ extreme short wȳded
