RN An Middleton 10 yeres old a week afore Miklemas sick miserablely handled w^th her fits. the 20 of decemb^r die ☿ at 6 in the morning. borne the 12 of Decēb. 1588. die ♃ betwixt 1. & 2 morning. her fathr {sic} enquireth what may be the cause of her disease. in her owne p^rsence. the 5 of feb: die ☾. h. 7. 30. ant m. 1599. of a white pale colour leane wellooking water very good. every day many fits. & on friday & weddensday. it maketh her knock stamp strick her hands a great cough /this figure I set in there p^rsence.\

RN Agnes Astill: otherwise Miller of Newport 40. yees {sic} old miserablely handled in her lims & ded poer & lowe brought. sent her this 26 feb: die ☾. h. 9. 15. mi. 1599. burneth & is full of blisters & itches.

RN M^res Androwes very sicke at the hart ready to die last night: water reasonable good full of scales & motes. sigheth mutch in her sleepe: se thincketh shee shall die suddenly. sent to me this 28 of feb. die ☿ h. 11. 15. 1599. looketh very pale & white. hath whit the {whites}

RN Iohn Moulsoe of Cosgrave 52. sick this fortnight of a grudging of the ague & troubled w^th a cough. looketh thin & yellowe w^th it this 28 of feb. die ☿. h. 12. 30. mi. 1599.

RN Goody Marcholl of Woulverton came to tell me howe her daughter was miserablely handled & cannot speake in her fits. 28 of feb. die ☿. h. 6. 15. 1599. sine consensu.

RN Margaret Marcholl & her husband came to me for some ease for her child of 15 yeres age 3 weekꝭ afore shrovetide. vppon a friday somewhat before daye. at 25 of feb: die ☉ h. 5. 15. m p m. 1599.

RN Iohn Barber of hanslop sent his water worst when he lyeth a bed. ill this fortnight 80 yeres a high coloured water & full of black colour.

RN Lawrence Mably of Hansloppe 72 yeres sick this moneth but keepeth not his bed. a sore paine in his backe. 27 of feb. die ♂ h. 11. 7. mi. 1599.

RN Mary Travell of Gothurste 20 yeres old not very sick but worse then she was maried 2 or 3 yeres since troubled at the hart y^t shee cannot goe no children sent this 28 of feb die ☿. h. 9. 15|6|. mi. 1599.

RN M^r foukes of Newport of 50 yeres old came him selfe w^th his wife for him selfe water mutch of a very good colour the 19 of febr. die ☾ h. 9. 50 mi ant m. 1599.
