RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Decēber. 4. \die ☾./ h: 9. 15. ant m. 1598 his hart \is good/ & his tonge talketh cheerf but his body swelled.

RN Iohn Risely of Rawnstyn 63|7|. {sic} 67. hath bene sick on weddensday last 6 weekes sound in body but bursten this 16 weekes. he was wont to be troubled w^th a wind Cholick. nowe last of l {sic} all w^th an issue on his navell eaten in an Inche deepe but nowe growen vp againe saving that it hath a litle hole in it w^c when they went to stoppe it put him in danger of death. sent his sonne to me he kepeth his bed nowe & then payned in his back & pricking & shooting in his navell {virs} an end. bleedeth every foote & wetteth a handkerchir. Decemb. 18. die ☾. h. 8. 40: mi. 1598. goeth ill to ground

RN Will: Seere of haversham. 53 yeres old sitteth vp by the fier, very stiffe & hardly to {ground} onc in 24 houres complayneth of his body & lims & legs most in on of his sides all over his left syde ever sinc March last: first amisse not able to goe over the Dore threshall. sinc Alhalloutide. sent his wife to me Decēb. 21. die ♃. h. 12. 45. 1598. on of his legs the left wonderfully swelled. a very diseased body & hath had a greate cough.¹

RN Iames Greenewood of water eaton Doct^r Sparkꝭ tennet {sic} 50 yeres old. complayneth of his of h an old dysease of his harte now well & lusty but not long sinc almost ded in it. troubled w^th a stitch on his right side & cometh over his hart ready to swounde. very cold w^th it. of a high colour. stuffed in his Chest y^t he can scarce tak his winde quærit Decēbr. 2. die ♄. h. 10. 15. mi. 1598. his hart trembleth muday {sic} last sick lame.

RN Goodman Honour of Newport for his wife who hath bene longe sick & is nowe very exceeding ill h {sic} as her husband telleth me. hanc male suspicant^r de veneficio

RN Iohn Risely of Rawnson sent onc againe to me for my counsell his pain lieth now in his Cods {w^c} are swollen as big as ons hed & as hard as a borde he was wont to haue them swollen very big but then lasted but 5 houres & nowe swelled ever sinc weddensday he cannot eate sleepe or goe to ground. troubled w^th the wind Cholicke a paine in his backe st{illeg}ry Desirous to take a stro{illeg} but Doubteth he ca shall be able Decēb. filius  patre cū consensu. RN Clement Nash servant to Iohn Connyngham 22 yeres old tooke her first on tuesday night qu about 8 a clock at night a crick {sic} \ache/ in her baccke \& lims./ y^t maketh her stoope & goe Double. troubled w^th a great heate of her stomack y^t it maketh her lips to break out he {sic} first water made in y^e morning red. the next water of a reasonable good colour. the wench of a ruddy face well colourd & fat. quait {sic} remediū Decēb. 4. die ☾. h. 12. 15. mi. 1598. of Newport

RN Edward fellow 24 yeres old of boy|w|ebrickle sick this moneth. walketh vp & Downe cōplayneth of his stomack hed & back. Decemb. 4. die ☾. h. 1. 10.

RN Iohn Smith of haversham. 36 sick ever this week kept his bed th yesterday all daye the 20 of decemb sent \on/ of his neighbours to me for his disease his water cold all white & muddy complayneth of his harte very sick 2i of Decēb. die ♃. h. 10. 1598. a fatt high coloured man

RN Elisabeth Smith 30 yeres old 9 yeres maried. never had Child it cometh by fits twice a weeke of a good colour red faced & fill {sic} of blood lieth in her hed & so ariseth from her hart & stomack vnto her hed. her face & syde swelleth. her vrine is red & full of rags. came in her owne son & then was very well of Decemb. 4 die ☾ h. 2. 1598. dwelleth at Emerten right side of her hed.¹

RN Edmund Hauckings of Newport 47|8| yeres old came to me reasonable well for ought I could ceave amisse & not sound in body any time 10 yees {sic} commonly at this time of the yeare but taketh him at Alhollantide & holdeth him vnto the springe of the yeare. came for my consell him selfe this 15 of Decemb^r die ♀ h. 9. 54. 1598. of a high reddish Colour of a black hed & beard. this disease hath greeved him a Chilly Cold in his hed.

RN Alce Denton of haversham 36. maried this 10 yeres 3 children last childe had a yere & more agoe feareth lest shee have taken a cold w^th a nurcing child an old Disease ever since shee was a servant a great payne at her harte, doth vomit ever sinc shee was maried {b} twice or thrice in a yere but very mutch this last weeke & never well vntill shee have it vp, els very sicke. a pert woman. sent to me by her husband this 17 of Decēb. die ☉. 9 – 18.. 1598.. best a bed.

RN Iohn Burman of Olny shephard lame in right leg & thigh this 3 weekes first in his back & then did shoote in his lims 37 maried this 17. yeres. Decēb 19 die ♂ h. 12. maried to a widowe had an ach in the other leg first many yers

RN vrsula Thorpe of Stony stratford keepeth the bull. hipwell vnkle vnto Thorpe 32 yeres old sick ever sinc mikelmas goeth abroade. Dece {sic} sent by M^r Oliv^r Barker. for his wife sister is her moth^r in frendship & kindred Doubteth wheath^r shee be bewitched & hath a canker in her tongue & tented shee hath an ill stomacke & spitteth putrifyed bloode. Decēb^r. 2i. die ♃. h. 9. 15. ant m. 1598.

RN Ellen Foster sore sick ever sinc her childe cannot take no drinke meate vomiteth vp all y^t shee taketh cannot quench her thirst swollen very big in her legs. now stolen vp to her hart on weddensday shalbe three weekes her child borne w^th out all greefe but mutch payned before & after & nev^r had Child before sent a woman to me for her Decēb. 31. h. 8. 35 m. 1598

RN Rich Kerby Miller of Newport 60 \56/ yeres old. long & grevously sick {filiu}s  patre \{servus}  Domino/ cū euꝯ {sic} consensu it hath held this 4 yeres by fits but this quarter of this yere held him sorely & keepeth home some days his bed in his fit & then abroad. a payne in his hart w^th a sighing black coloured very fat of his face but w^th this sicknes shrunk in his bodye & legs. M^r yarrow for him novemb. {sic} 1 Decēb. die ♀ h. 10. 15. 1598¹

RN Iohn Malins a yere & a quarter lacking a fortnight as it were pined in all her lims & bodye hed a poer bare creature. cannot sleepe not at any time in a moneth or fortnight but crye and Drinke mater adduxit se{cū} filiolā veret^r ne sit maleficio v^l veneficio afflictus. Decēb. 7. die ♃ hor. 3. 30. mi. 1598 very short winded.

RN Frauncis Marshall of Newport very strangly can scarce speake, only maketh signes for water to coole her her mouth is furred white w^th extreeme heate taketh her Drinke by a quill. first bound but y^t I gave her vnbound her nowe in a laske & extreeme hot & drye Decēb. 12. die ♂ h. 9. 30. mi ant m 1598. troubled in her sleepe.
