RN Amy Hoding of Addington 18 yeres of age. never had her couse {sic}. lyeth in her hed & chest & soppeth {sic} her eares. swollen in her feete & morning in her face & & eys & avoydeth bloode vpward d|A|pr. die ☾ h. 2. 15. m p m. 1599.

RN Richard Kinge of Nash. 24 yeres hath had a swelling in his body belly & mēbers & face for this 3 yeres & goeth away agayne. Octob. 3 die ☿ h. 12. 45. {illeg} 1599. hath bene at Cublington son of Thorton’. in his throate & k {sic} & knees.

RN at this hor {h} moved me for his wife at 1 of the clock for to kill & destroy her chest wormes./

RN Edward Walduck craveth my helpe helpe {sic} further for his the ase ease of his belly & chest of w^c he still cōplayneth although he have found some ease by the purgat I gave him w^ch gave 10 stoles or thereabout god be thancked for it. Aug. 4 die ♀ h. 11. 46|9|: mi

RN Iohn Malins a yere & a quarter lacking a fortnight as it were pined in all her lims & bodye hed a poer bare creature. cannot sleepe not at any time in a moneth or fortnight but crye and Drinke mater adduxit se{cū} filiolā veret^r ne sit maleficio v^l veneficio afflictus. RN Iohn Phylip of Bletchly 50 mendeth not after his phisick y^t I gave him for it wrought w^th him but a litle cōplayneth still of his body & hart die ♂ Decemb 4|5| die ♂ h. 11. ant m. 1598. 11. 30. m.

RN at this time Mary Pen of Straford sent me her water. w^ch was full of motes & scales & had gravell in the bottome. her former phisick I gave her wrought vpward. hath a great gydines at in her heade hart feeble bellye.

RN Henry Lea. of Bechampton’ 28|9|. of Sshoullbroad lodge in Northampton shere for his sore red eye & hed. what is best for it. Novemb^r 24 die ♄. hor. 10. 30. ant m. 1599. 16. yeres hath lost his sight w^th an arrowe.

RN Henry lea 29 y of bechampton. his phisick did not worke Decēb^r 13 die ♃ h. 11. pater sine consensu filij paynd in his eye & head. & his {rhee} rheume & ey rūneth. ant m. 1599.

RN Margery Clarke a midwife of Stony stratford for a great swelling in her thiges & bo legs y^t on may see her face in it & so striketh vp to her harte ready to stop &|h|er winde. the bell went for her yesterdaye hath a great scouring this on her & hath had it a longe time.

RN Thomas Gardner of Dunsanger of 62 yeres old y^t pissed blood & was mended by the herbꝭ y^t I sent him in a receipte. sinc y^t time had on Waly who hath w^th his Clisters Done him litle good but is swollen in his belly w^c is growen very harde. & mutch payned in his share & can make but a litle water halfe a sponefull at a time would knowe wheath^r leekly to die or live. went not to ground sinc munday last about 12 a clocke sent to me this 18 of Apr. die ☿. h. 7. ant m 1599 burneth in his yarde.

RN Bartlemew lea of Padbury. 36. married. sent agayn for phisick sick everywher. his former phisick wrought a litle but he nev^r the better Aug. 20 die ☾. h. 9. 15 ant m. 1599.

RN Mres Androwes of Broughton sent to me againe the 13 of decēb. die ☿ h. 9. 30 ant m. her former phisick I gave her wrought well w^th her. 1598. it cometh out of her mouth in great abundanc leeke cleare hot water.

RN Katheron Dudly of Woulson 53. for the paine in her belly taketh it to be the Cholick. if shee cast shee is well. & doe w^th all avoyd wind. came to me againe for my counsell 27 of Marche die ♂ h. 8. ant m. 1599.

RN Ihone Clarke of Rawnson 63. sick this sennet. cannot rest in her bed can tak no rest was pretely mended & now ill againe in her chest./ this 18 of Decemb. die ☾ h. 10. 15. 1598.

RN Goodmā|dy| Nueman of Brodwell. of 45 yeres of age found mutch ease by y^t w^c phisick w^c was given her & wrought vpward & downeward w^th her & shee was very prety well after it for 2 or 3 days. &|b|ut st the thirde day after it tooke her agayne & she is mutch troubled w^th the winde cholick & w^th a hot & drye stomacke. & sent her daugh{t}{er} agayne to me. Aug. 26. die ♄ h. 12. 30. 1598.
