RN Mres Haynsworth of Stonystratford 35 y & as mutch as frō the 4 of August last was strangly taken at the hart & did swound often tymes almost Dead Decēb. 9. ☿ h 12 1618.

RN at this time Mary Pen of Straford sent me her water. w^ch was full of motes & scales & had gravell in the bottome. her former phisick I gave her wrought vpward. hath a great gydines at in her heade hart feeble bellye.

RN Agnys Thorne of Wavendon servant to Goodmā Hull. 30 y. novemb. 6. die ♃ h. 10. 45 ant m 1600 back reanes right syde harte swoneth w^th it.

RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke.
