RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN Rich Kerby Miller of Newport 60 \56/ yeres old. long & grevously sick {filiu}s  patre \{servus}  Domino/ cū euꝯ {sic} consensu it hath held this 4 yeres by fits but this quarter of this yere held him sorely & keepeth home some days his bed in his fit & then abroad. a payne in his hart w^th a sighing black coloured very fat of his face but w^th this sicknes shrunk in his bodye & legs. RN Robart West of Haversham for a payne in his loynes & hips & right leg & toes ever since Sunday last at evening at 4 a clocke & hath had a winde cholicke ever since he was a batcheler & his wife also ever sinc a meaden hath bene troubled w^th a gnawing in her body. 56 yeres old. came frō Sherington. RN Katheron Allen at Newport 50 yeres old. amisse this {illeg} yeres assone as shee was delivered of a child paynd in her hed. & swollen in her face. aremes {sic} legs hoven in her body sweateth & her hed & neck & syde loketh very hie & is specled. short winded & faynte. March 27. die ♂ h. 11. 30. 1599

RN Mres fayth Gybbes 50 sent to me againe the 5 of May. for a stiffnes in t & swelling & knobbes in t her wrest & inside of her wrests & hands, for a paine in her sinewes an {sic} an ache in her bones, hands & knees. altering from place to place. die ♄. h. 10. 30 m ant m. 1599.

RN Agnes Rutland of Newnton Longfild. 53. sick this day fortnight. payned in her hed harte & all the body over. cannot abide vp: costive onc in 2 or 3 days. maij 1. die ♂ h. 12. 10. m. 1599.
