RN M^r Neale of Drury lane in london enquired of me Decēb 23 ♂ h. 8 ant m 1617 whenc it was y^t on swounded 3 or 4 houres vppon any frigiht {sic} or disc wheath^r it came frō the spleene or noe or whenc I set {this} after he was gone at nine but he enquired at     ant m 1617¹

A greiuous Headache takes him at Midnight sometimes in one te of y^e head & sometimes in y^e other te, but especially in y^e left side of his heade, w^ch makes y^e places outwardly sore where y^e paine hath beene.¹

A continually Noyse in his head w^th a tearinge paine as yf it would pull one skyn from another, w^ch comes sometimes suddenly like a strech, & then is gone in an instant

A dizines in y^e heade & a leapinge, as it were of blacke mothes before his Eyes

This paine hath continued these 6 weekes,

Desires to knowe whether it ceeds from y^e stomack or spleene, from a cold or{illeg} hote cause

Hath beene let Bloud in y^e right arme, & vnder the tongue, & in y^e forehead, beeinge tolde by the Physitian y^this greife was caused by fullnes of Bloud

Hath taken Pills & Electuaryes sundry times, but his paine noethinge deminished

Hath for many years beene troubled w^th a soundinge & noyse in his head, but neuer w^th paine till nowe Feareth a Disposition to the Palsye or Apoplexye Desires Receits & Directions to preuent these two capitall Dangers.

GJ M^r Adrian Fortescue and m^rs Mary Fortescue have taken thrice of their physicke and wrought well w^th them, the gentlewoman sicke the first taking, and well since, her stomack better, but her head as it was

M^r Adrian sicke w^th his physicke on monday last and tooke none since he sleepes not well, is growen weake and his waxen kernells remayne. / my Lady thinkꝭ that if he continue so it will bring him to a consumption She desyres to have some course for m^r willm Fortescue 10 y. w^ch hath also an Itche. Desyres to be satisfyed how this Itche comes whether by overheating the bloud w^th violent exercise. & whether they shall continue the rest of the physike.

RN Ralfe Vines is troubled w^th a gydines in his head y^t & clappeth over his eyes. & maketh him not to knowe what & {sic} maketh him very Drowsy a youth 13 yeres old at miklemas next. enquireth a remedy & of what it is caused Sept. 1{5}|6| die ♄. horr 9. ant m. 1598.
