RN Susan Blundle at x yeares old at the 17 of Septemb. last was on the 29 of Ian at night sore troubled. & in amaner trice ded in a sownd & is every satterday so troubled at night specially Susan Blundle in her bead & sometimes in the morning. it first tooke her in the 23 of {sic} 29 day of Ian. at midnight in 1597. th 29 of Ianary. |after midnight. 30 mi.|

RN Thomas Barret of stony stratford servant to Iohn Smith of haversham tayler {vousued} to nothing extreeme weake cannot stirre out of his bed. the 28 of Decēb. die ♃. h 9. 30. m. ant m. 1598 pater  filio.

RN at this time Mary Pen of Straford sent me her water. w^ch was full of motes & scales & had gravell in the bottome. her former phisick I gave her wrought vpward. hath a great gydines at in her heade hart feeble bellye.

RN Mary Nuby 28 of Grafton Lodge. for a rising at her stomack. her legs. Double vnd^r her faynt & feeble & pineth in her body & yet can earte {sic} had not her termes never sinc her sicknes almost for a yere. Ap. 25 die ☿ h 5. 30. p m. 1599 |cannot sleepe troubled w^th a great cough at night.|

RN Iohn Scriven of Hanslop. 46. a maried man had|th| a disease y^t holdeth him as an ague. & mutch Disquiteth him in his belly chest & head. very faynt & feeble & Did sweate mutch could not tary vp frō his bed a great griping in his belly Ap. 28 die ♄ h. 10. ant m. 1599.

RN Arthur Smithe of Woulverton. 36. payned in the bodye nowe & lims & knees. & hath a great hote stomack and mutch bounde. arms this 7 yeres & hath spent mutch monye. faynt & feeble. maij 1. die ♂ h. 9. {illeg}|3|0. ant m. 1599.

RN Harry Barton of NorthCrowlye of the east end Aug. 20 die ☾ h. 12. 20. 1599. keepeth his bed very weake & sicke.

RN Susan Blundle {illeg} 1597 was grevously handled w^th a disease w^ch taketh her every saterday since it began to take her w^c was not vntill this yeare 1597 & towards the latter end of Ian. shee was greeved therw^th on feb. the 12 of feb{r} betwixt on & two in the mor:¹ shee is as on dead & sowneth 2 or 3 times togeath^r. or a litle after midnight betwixt 12. & on. it tooke her cōmonly in her sleepe./

RN Ralfe Vines is troubled w^th a gydines in his head y^t & clappeth over his eyes. & maketh him not to knowe what & {sic} maketh him very Drowsy a youth 13 yeres old at miklemas next. enquireth a remedy & of what it is caused Sept. 1{5}|6| die ♄. horr 9. ant m. 1598.

RN Thomas Berill 6 70 of Yardly Hastings troubled w^th a coughe mutch fleume y^t he wetteth every night 2 cloathes. faynt: Aprill. 3 die ♂ h. 10. 30. ant m. 1599.

RN Mres Edmunds of Shenly for a windcholicke. & for a swiming in her hed. may the 18 die ♀ h. 2. 27 m p m. 1599.

RN Goody Fisher sent her water to me to know what was good for her Aug. 14. die ☾ h. 7. 45 shee is troubled w^th the mother & was wont to swound w^th it very often. at 7. 30. 20.¹

RN Iohn Franckling of dreiton 60 yeres sick this quarter of the yere in his harte. & chest. Martij 1. die ♃ h. 3. 30. m 1599 so weak he cannot goe out a dores. 1599.

RN M^r Longvile of {sic} Brodwell of Woulverton for his disease as he had a fever. 26 of March die ☾ h. 10. 30. ant m. 1599 a fayntnes & panting of the hart

RN M^r William Chester was borne vppon Easter monday about 4 of the clock p m. this Child is f {sic} very small faynt feble of a longe visage hath not the vse of his lims but specially of his legs to goe natus āno dni 1596. 12 Apr. die ☾ h. 4 p m. 1596.

RN An Bourton of Olny 26 yeres old. sick the ever sinc Lent tooke her in her hed hart & backe maried this 7 yeres had but on Child but y^t god hath taken to his mercy. {s} {sic} it holdeth her nowe as an ague shaking her every day. & hat sweateth & is very hot after the shaking & hath as it were a thing lūbring in her chest very corpulent faynt & very feeble. hath a very painfull cough ready to Drawe vp hart & hall w^th it & fetcheth blood very short winded. fetcheth vp fleume & water as cleare as a bell. Ap. 25 die ☿. h. 8. 30. mi ant m. 1599. hath a very sore ague every day y^t maketh her sweate exceedingly. hath not had this moneth her Courses sinc shee voyded a litle porreng^r of clompered bloode/

RN Edward Newnton sick this 14 weekꝭ of Stony stratford a weaver hart & everywhere sick pined & yet eatet skin & bone. & yet will eate any thing. 60 yeres old. faynt. hard bound {illeg}|y|et onc in 24 houres. feareth lest he be b{edr}ed a drie coughe. this 8 yeres. Aprill 3 die ♂ h. 10. 1599. reareth fleme, in a cōsumption. hart hele & stronge.
