RN Rich Kerby Miller of Newport 60 \56/ yeres old. long & grevously sick {filiu}s  patre \{servus}  Domino/ cū euꝯ {sic} consensu it hath held this 4 yeres by fits but this quarter of this yere held him sorely & keepeth home some days his bed in his fit & then abroad. a payne in his hart w^th a sighing black coloured very fat of his face but w^th this sicknes shrunk in his bodye & legs. RN Iames Greenewood of water eaton Doct^r Sparkꝭ tennet {sic} 50 yeres old. complayneth of his of h an old dysease of his harte now well & lusty but not long sinc almost ded in it. troubled w^th a stitch on his right side & cometh over his hart ready to swounde. very cold w^th it. of a high colour. stuffed in his Chest y^t he can scarce tak his winde quærit Decēbr. 2. die ♄. h. 10. 15. mi. 1598. his hart trembleth muday {sic} last sick lame.

RN Ihon Phylip at Bletlhy {sic} 42. yeres a married man sick th but this weeke sick very mutch in his body. made his will yesterday sent him self this 3 of Decēb. die ☉ h. 2. 1598. he hath not gone to ground never sinc he sickned

RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. Decēber. 4. \die ☾./ h: 9. 15. ant m. 1598 his hart \is good/ & his tonge talketh cheerf but his body swelled.

RN Iohn Connyngham 7 yeres old satterday sennet after miklemas daye sick this 12 moneth. cōplayneth most of his hed & hath a weake stomack. was wonte to have white water leeke dregs of a tub. very thick of a pale & whitish Colour. a longe visage. Decēb. 4. h. 11. the child came hither.

RN Edward fellow 24 yeres old of boy|w|ebrickle sick this moneth. walketh vp & Downe cōplayneth of his stomack hed & back. Decemb. 4. die ☾. h. 1. 10.

RN Thomas Dudly nowe 5 yeres old next miklemas natus erat Sept 15. die ☉ h 1. p m. hath his health well & eateth his meate. about harvest last began to have a rising in his throte ready to stop his wind & speach in always when he beginneth to sleepe then it hath held him sinc miklemas the last time it tooke him it bete him on friday last. going to sleepe. ready to skreame & skrike in the fit.

RN Iohn Bynion natus 1595. die ☾ 29 septēb. h 2. 30. ni. {sic} p m. 1595. troubled exceedingly w^th hed ache. & will eate but litle.

RN Iohn Phylip of Bletcley very extreeme sick bound & very Costive the last phisick would not work w^th him as Doct^r Spark writeth me, nowe again sendeth to me. this 9 of Decēb^r die ♄. h. 12. 15. 1598. troubled w^th the wind cholicke
