RN Susan Blundle {illeg} 1597 was grevously handled w^th a disease w^ch taketh her every saterday since it began to take her w^c was not vntill this yeare 1597 & towards the latter end of Ian. shee was greeved therw^th on feb. the 12 of feb{r} betwixt on & two in the mor:¹ shee is as on dead & sowneth 2 or 3 times togeath^r. or a litle after midnight betwixt 12. & on. it tooke her cōmonly in her sleepe./

RN Susan Blundle at x yeares old at the 17 of Septemb. last was on the 29 of Ian at night sore troubled. & in amaner trice ded in a sownd & is every satterday so troubled at night specially Susan Blundle in her bead & sometimes in the morning. it first tooke her in the 23 of {sic} 29 day of Ian. at midnight in 1597. th 29 of Ianary. |after midnight. 30 mi.|

RN Katheron Ardes of 2i of Sherington hath to mutch store of her naturall Diseases & make her very fainte & feeble come in cl{om}tons Ap. 26 die {illeg} ♃ {sic} h. 12. 25. m.

RN Elisabeth Saule 31 yeres her child a yere old. amisse 3 days. but grudging this fortnight very weake & faynte somewhat sanguine Apr. 5. die ♃. h. 10. 23. ant m. 1599.

RN Mary Rive came to me for the greene sicknes w^c made her very faynt Sep{t} 21. die ♃. h p m 5 30 1598. of 18|6| yeres old. 16 y

RN Mary Nuby 28 of Grafton Lodge. for a rising at her stomack. her legs. Double vnd^r her faynt & feeble & pineth in her body & yet can earte {sic} had not her termes never sinc her sicknes almost for a yere. Ap. 25 die ☿ h 5. 30. p m. 1599 |cannot sleepe troubled w^th a great cough at night.|

RN Ralfe Vines is troubled w^th a gydines in his head y^t & clappeth over his eyes. & maketh him not to knowe what & {sic} maketh him very Drowsy a youth 13 yeres old at miklemas next. enquireth a remedy & of what it is caused Sept. 1{5}|6| die ♄. horr 9. ant m. 1598.

RN Alce Masun{n} of Woulverton 14 yeres old. took her vppō wendesay {sic} the 2i of febr: betwixt 2. & 1: shee nothinge but shaketh & soundeth as they think w^th an impostume. die ☿. 21. of feb. 1599. came her moth^r moved her quest. the 24 of feb die ♄. p m. 6. 38. 1599

RN Mres vuedale for her selfe came to me for my counsell 1598 the 10 of Iune die ♄. hor. 10. ant merid shee is faynt & hath had a laske.

RN An Goodrick of Lathbury 32. hed & neck pricking left side her hed was gyddye. shee had a child 9 month a goe troubled w^th a gydy hed. Novemb 2. die ♃. h. 3. 30.

RN Alce Phylip of Eaton 60 yeres old sick sinc St Steaphens day at night. betwixt 9 & 10 at night. sent to me being very weake this 1 of Ian 1599. troubled w^th a stitche vnder her side.

RN M^r William Chester was borne vppon Easter monday about 4 of the clock p m. this Child is f {sic} very small faynt feble of a longe visage hath not the vse of his lims but specially of his legs to goe natus āno dni 1596. 12 Apr. die ☾ h. 4 p m. 1596.
