RN Vrsula Thorpe of Stony stratford 34 sick sinc mihelmas cōplayneth most of her mouth. & of a canker therin & had a tente keepeth nowe her bed this sennet & so did not of{illeg} sent her water by her frind Decēb. 23. die ♄. h. 11. 15. 1598.

RN Ioys Armfild of lathbury 50 yeres old sick first on the 23 of Decemb. {illeg}a{illeg} sent her husband to me on weddensday the 27 of Decemb. die ☿ h. 7. 45. ant m. very sick last night & keepeth her bed w^th it. her husb. had on wife before & by this 6 children 1598.

RN Thomas of Lowton servant to M^r furze. 23 yeres old very sick & leekly to dye fell sick vppon St Thomas daye towards night sent to me this 29 of Decēb die ♀. h. 10. 30. ant m. 1598.

RN Ihone ford of Dreiton Parsnā. 54. sick this twelmoneth. but keepeth her bed for this 8 days. the 20 of decēb. sent to me this 29 of decemb h. 11. 6|9| m. die ♀ 1598. water betwixt yellow & white being cold.

RN Iohn Risely of Rawnson sent onc againe to me for my counsell his pain lieth now in his Cods {w^c} are swollen as big as ons hed & as hard as a borde he was wont to haue them swollen very big but then lasted but 5 houres & nowe swelled ever sinc weddensday he cannot eate sleepe or goe to ground. troubled w^th the wind Cholicke a paine in his backe st{illeg}ry Desirous to take a stro{illeg} but Doubteth he ca shall be able Decēb. filius  patre cū consensu. RN Ellen Foster sore sick ever sinc her childe cannot take no drinke meate vomiteth vp all y^t shee taketh cannot quench her thirst swollen very big in her legs. now stolen vp to her hart on weddensday shalbe three weekes her child borne w^th out all greefe but mutch payned before & after & nev^r had Child before sent a woman to me for her Decēb. 31. h. 8. 35 m. 1598

RN Alce Phylip of Eaton 60 yeres old sick sinc St Steaphens day at night. betwixt 9 & 10 at night. sent to me being very weake this 1 of Ian 1599. troubled w^th a stitche vnder her side.

M^r Knifton of Lowton 55 yeres old sick this 3 weekes {illeg} sunday last first in his stomack w^th an extreeme cold w^c tooke a way his stomack ache in the head neck & legs sweateth div^rse times & after troubled w^th an itch as exceeded greatly onc in two or 3 days to grounde. nev^r vomiteth. the 3 of Ian. die ☿ h. 9. 15. 1598. {sic} 1599. bad head greefe taken {in} lawe.

RN Itm’ Rich. stonedale troubled w^th a feare & a scriching. in the hed & hart called for a candle at night & d {sic} prayed & desyre his fathr {sic} so to doe bound 2 or 3 dayes Ian. 3 die ☿ h. 10. 30 1599.

RN Goody foster sent to me Ian. 3. die ☿. 10. 3|1|5. \30./ mi. ant m. troubled at her hart & chest her water full of black scales & blacke humoures.

RN M^r {sic} Charels Tutcher Charles at Stonystratford at the swan of 2 neckes. 45. sick this fortnight. sonewhat {sic} Costive shee doth bleede & is very mutch stuffed in her chest can scarce tak her winde. ready to sound a grosse & fat woman. sent a poer woman.
