August the 12 die ♄ h. post m ant m. 8. 34 mi for Agnes Miller who hath bene troubled w^th the rūning of her reanes & so also her husb: for the space of this 4 yeares & her mother w^th the winde cholick. whiles she had her termes shee was a sound woman but sinc never having thē shee hath bene mutch distempered w^th the rūning of the reanes

RN Rich. Risely of Willing 2 yeres old. the fortnight after Chr|E|sterday ☉ morning about 3 a clock morning troubled by fits. especially the 18 of Decēb. after noone it wull {sic} fall Downe & crye & tak hold of any thing. Richard Risely a very feare child & of a goodly cōplexion was borne 1597. the 10 of Aprill. die ☉ h. 3. ant m. 1597. came to me Decēb. 29 die ♀ – h. 9. 30. cū vxore et filiolo. borne at willing.

RN M^r furyes servant of Louton very extreeme sick at hart cannot abide talk a water leek the yellowe iaundes. sent to me againe this 4 of Ian. die ♃ h. 8. 52. mi 1599. his former phisick wrought but litle. never vomited but to the ground his m^r {sic} thinketh he hath the yellow iaundish. his fath^r moved a question at the same time or a quart^r after 9. for a payne in his knee w^c was broken his thigh was broken quite thorough & nowe the paine is fallen into his knee pan & is swollen.

RN Guy Foster of haversham 10|i| yeres old last Christmas came him selfe w^th his mothr {sic}. borne on a friday Martij 12. die ☾ h. 10. 15. 20|3|. 1599.

RN Ione C {sic} Kittre of Broughton. for her disease this 14 of March. die ☿ h. 3. 10. mi. 1599 30 yeres her child 10 weekes old.

RN Richard Lingard this a yere & halfe old. troubled w^th an ague & w^th the wormes. & leekely to have the iandes the child is weaned sent to me by there sister. the fath^r & the moth^r boath Apr. 17. die ♂ h. 2. 30|5| p m. 1599. of Simpson a great cold.

RN Alce Clarke of North Crowly about 50 48. Ap. 20. die ♀ h. 4. 1599 amisse this fortnight.

Will Clarke not well in his chest & harte very short winded.
