O most mighty & most powerfull god, who hast ever bene exceeding glorious in thy sainctꝭ & servantꝭ, enduing them w^th vertue celestiall & power divine as thou didste some time Elisa thy servant, who by thy vertue cured Naaman the leper & moyses who healed miriam his sister of her leprosy, & gavest vertue & power to thy holy & blessed d|A|postels & Disciples |{illeg} \boath/| for \to heale diseases & to cast out divels also for/ the confirmation of thy gospell & advancement of thy glorious name, give vnto me thy vnworthy Disciple & hūble suppliant, for vnto thee nothing is impossible, give vnto me (my Deare sufferaigne & my leach lord) that gift divine & sunaturall y^t vertue celestiall na sucelestiall that power from above, y^t whatsoever thy hūble servant craveth & beggeth of thee his Lord \& sufferaigne/ In the name \of/ thy sonne Ihesus Christ, the same may be graunted to me him. y^t whose sins we thy vnworthy ministers & disciples remitte, they may be remitted according to thy mise which thou madest \& gavest/ to thy apost: saying whose sins ye remitte they be remitted etc’ & whose sins ye retaine they be retayned graunt \give most Royall kinge that vertue & power frō above/ we hūbly praye thee, y^t whose disease whomsoever of our flock thy vnworthy shephard intendeth to heale in thy\e/ powerfull name of thy beloved sonne, Ihus. he may be healed. & further grant y^t if any of our flock be \being/ haunted by any \an/ vncleane spirite or spiritꝭ, may by our service & ministery & by thy vertue Royall given to vs & left with vs by vertue of our ministery graunt I say y^t if in thy\e/ name of Ihus we cōmande him or them \or any of them/ to dete, they may p^rsently &fourthwith at \& vppon/ the mention therof soddenly departe.

O most mighty & most powerfull god. who hast ever bene glorious in thy sainctꝭ \servantꝭ/ & children enduing them w^th vertue \celestiall/ & power \divine/ from above, soe y^t many of them {sic} \as thou sometimes/ have done. \endued {illeg}/ many things straung & Wounderfull \thinges/ in the eyes of the world. as \assometimes thou didst/ Eliseus {sic} \Elisa/ who cured Naaman of his leprosy, & Moyses who cured myriam his sister of her leprosy, & gavest vertue & power to thy \holy & blessed/ apostels & Disciples y^t whose diseases they would have to be cured should p^rsently be healed & cured. \that they might heale all maner of diseases whatsoever/ graunt \give/ vnto me \I we pray thee/ thy vnworthy Disciple y^t whose disease /a\ \heavenly/ gift & vertue y^t whose \no/ disease is by vs & our ministery \may be soe deepely/ rooted in any \of thy servantꝭ & children/, but y^t at our \hūble/ request & \yernest/ suite \made/ vnto thy maiesty it may be removed & the ty diseased may be cured & healed & restored vnto the health of body & minde. this favour graunt th me thou god most gratious god. thorough Ihesus Christ our lorde.

In all quiet & peacable maner w^thout harming or annoying of any \of thy creatures/ As they did when thy sonne \C. Ihesus/ him selfe cōmaunded thē & thy apost. & Discip. Peter & Paule chargded thē in the: name of Ihus. to whō all things in heaven & yearth & vnder the yearth doe bowe & yeld there obedience. this gift lord we beleeve thou canst give vs, because thou art omnip. & almighty. & all things in heaven & yearth obay thy vertue. & power. & because we knowe & beleeve thou hast given this gift to many of thy servantes As to thy Discip. & apostels by name & also hast \mised/ that thou wilt leave that gift \of curing & healing of infirmityes/ & power of ca {sic} casting out evill & vncleane spirites vnto thy church for over {sic}. this lord graint vs to doe for Ihus C. his sake. scio dne Ihus qd oia credenti sunt possibilia, aduivat et adauge fidem dne servit ne in oe \suscepto v^l suscipiendo A/ vnquā deficiat qꝛ tua est laus honor potestas \gloria/ potentia in secula scloꝝ.
