RN Iohn Persivall of 56 yeres \of Buckingham./ of 56. complayneth of his head & hart & stomack & would fayne have some ease. Sept. 7. 40. {sic} & hath bene sick in this disease for this 6 weekes. & this 3 yeares & maketh his stomack swell 1598. die ♃ h. 7. 40. ant m.

RN Ralfe Vines is troubled w^th a gydines in his head y^t & clappeth over his eyes. & maketh him not to knowe what & {sic} maketh him very Drowsy a youth 13 yeres old at miklemas next. enquireth a remedy & of what it is caused Sept. 1{5}|6| die ♄. horr 9. ant m. 1598.

RN Edmund Hauckings of Newport 47|8| yeres old came to me reasonable well for ought I could ceave amisse & not sound in body any time 10 yees {sic} commonly at this time of the yeare but taketh him at Alhollantide & holdeth him vnto the springe of the yeare. came for my consell him selfe this 15 of Decemb^r die ♀ h. 9. 54. 1598. of a high reddish Colour of a black hed & beard. this disease hath greeved him a Chilly Cold in his hed.

RN vrsula Thorpe of Stony stratford keepeth the bull. hipwell vnkle vnto Thorpe 32 yeres old sick ever sinc mikelmas goeth abroade. RN Thomas Leedes hath a great payne greeping his guts doubteth he tooke it w^th a fall on his ribbes. came to me for my Comsell {sic}. 75. maye the 10. die ♃ h 10. 12. ant m. 1599 cold in the morning but very hot after noone./

RN M^r George Burye for his wife troubled w^th melancholy & sudden passions. troubled w^th windines & payne in her stomacke & dissynes in her head. May. the 12. die ♄. h. 11. 15 ant m. 1599.

RN Oliver Hames of his sonne maried a great payne in his stomacke & arising ready to stop his wind. 30 yeres may the 13. die ☉ 40 h./ 7. 40 ant m. 1599. had a bruise w^th a horse did spit blood cam̄ot {sic} stoope to work for it

RN Michaell Simpson of Woulverton 40 y. August 22 die ☿ h. 11. 15. ant m. 1599 it cōmeth on him as an ague payneth him in his chest back head. feareth wormes because he hath bene sore tormented.
